Requests for letters of recommendation
Please use this form to give me all the information I'll need to write you a strong letter of recommendation. Fill out all the info, then also make sure to email me to tell me (1) that you requested a letter and (2) when the deadline is.

Except in unusual circumstances, I will not be able to write a letter unless you give me at least 2 weeks notice before the deadline.

Please remind me about your request one week before it is due. I will email you to confirm once I've submitted your letter. If you don't hear from me, make sure to send me more reminders!!

Some kinds of opportunities people apply for have diversity/inclusion efforts. Would you like me to address any of this in my letter? And if so, how would you like me to phrase it / how do you identify? This might be your ethnicity (describing feelings of belonging and attachment to a distinct group of a larger population that shares their ancestry, color, language or religion), whether you are the first person in your immediate family to pursue higher education ("first generation college student", etc), veteran status, disability status, or sexual orientation. You are not required to disclose any information here that you are not comfortable disclosing! (See below about mentioning anything you do not want me to discuss in the letter).
