ASC 2025 Select Team Tryout Waiver Request Form
Due to the large number of players involved, ASC does not offer makeup dates for tryouts. If your child has an injury or unavoidable conflict please STILL register them for tryouts, and submit the below request for a potential waiver. 

Waivers are given for injury, illness or long-standing commitments such as a performance or family graduation. Due to the complexity of schedules around known school-based activities and events, we do not grant waivers for other sporting events or tryouts. While waivers are given on a discretionary basis, it is best for players to be at both tryouts whenever possible.

Submitting this request does not automatically provide players with a waiver.  The ASC Tryout Committee reviews all waiver requests and will reply to your submission in a timely manner.
