By registering for the Challenger League you agree to the Waiver below. WAIVER RELEASE OF LIABILITY I hereby for myself, my heirs, executors and administrators waive and release any and all rights for damages I may have against the Town of Franklin, Franklin School Department, Putnam Investment Company, FYBO, Camp Haistan, or any other venue public or private, that may hosta sanctioned FYBO event, their agents, representatives and assigns, for any and all injuries suffered by myself, my child/ward while participating in the Franklin Youth Baseball program. I also assume all risks and hazards incidental to such participation including transportation to and from the activities; and I do hereby waive, release, absolve, indemnify and agree to hold harmless the Franklin YouthBaseball Organization, Board Members, sponsors, supervisors, participants and persons transporting my child to or from activities, for any claim arising out of an injury to my child, whether the result of negligence or for any other causes, except to the extent and in the amount covered by accident or liability insurance. PROGRAM ACKNOWLEDGMENTS I acknowledge receipt of a copy of the “FRANKLIN YOUTH BASEBALL CODE OF CONDUCT, 2023" as well as a copy of the “RELEASE OF LIABILITY” and “PROGRAM ACKNOWLEDGMENTS” statements contained on the, “FRANKLIN YOUTH BASEBALL ORGANIZATION 2007 REGISTRATION AND RELEASE FORM”. I pledge to read the Code of Conduct and to comply with all its terms in an unconditional manner. I will ensure, that those player(s) that I have registered, read and understand the Code of Conduct. I pledge that all spectators attending an FYBO event in support of a player that I have registered will be properly advised of the code of Conduct and its terms. I understand that the player will be assigned to an age group as defined by league rules and procedures. I hereby give my approval for the player listed above to participate in any and all FYBO program activities. I agree to return equipment issued to my child in the same condition as when received except for normal wear and tear. I understand that fees are non-refundable from Franklin Youth Baseball Organization after March 1st. I have read and freely agree to the aforementioned releases and acknowledgments.
