Complete this form to enrol for the Monday Hat League hosted by PRIMAL Ultimate Club at Yokine Reserve in Yokine.

ONLINE REGISTRATIONS CLOSE FRIDAY MAY 7th 2021.
Individual registration format, your name goes into the “hat” each week and teams are selected randomly. Your team will change each week so you get to play with and meet different people. Teams are mixed gender and a mixed level of experience.
When: Mondays 6:30pm-7:30pm
Duration: Six-week league - May 10th to July 14th.
Cost: $30 adults and $20 for juniors (under 18 years old)
Who: Men, Women and Youth (12 yrs+)
