CSEAP Counseling Appointment Request 
Please request an appointment for counseling (individual, group, couples) by completing this web form. CSEAP may require up to 2 business days to respond to your completed form. 

If you are requesting response to an urgent mental health or personal crisis, please call 303-866-4314 for more immediate response. 

State employees have access to 8 confidential counseling sessions per rolling year. State employees have access to 3 professional coaching sessions per rolling year.

State law requires that CSEAP counseling clients are physically located in the State of Colorado. 

We are unable to provide counseling when a party or parties are temporarily or permanently located outside of Colorado.
