Upon submission of this form, you are committing to your chosen method of instructional delivery until the end of the school year. If the desired method of instructional delivery is different from your child's current situation, no changes will take place until after spring break on Thursday, April 8.PLEASE COMPLETE THIS FORM BY FRIDAY, MARCH 5.
