ACKNOWLEDGEMENT:  I have read the USD 231 "Out of District Transfer Request" procedures.  In addition to completing this form, I am providing all required supplemental material.  I understand if this request is approved by the USD 231 Transfer Committee, the student granted the out of district transfer shall be admitted on a per-semester basis.  I further recognize this is a voluntary arrangement between the parent/guardian and the school district and shall be reviewed bi-annually (each semester).  The parent or guardian must complete and re-submit to the Clerk of the Board of Education an "Out of District Transfer Request” form on an annual basis.  The committee will consider availability of space and other factors, including, but not limited to, academic progress, attendance and behavior/discipline records in making a decision on the application.  Lastly, I understand that approval, regardless of “Reason for Request”, is not guaranteed and that USD 231 reserves the right to cancel, revoke, or rescind approved requests at any time. *
