Lawson PTA Honorary Service Award (HSA) Nominations
Every year
Lawson PTA
and the Cupertino-Fremont-Sunnyvale Council of PTAs honor volunteers and school staff who have made significant contributions to the well-being of children and/or families at Lawson and our community. We need your valuable input in recognizing and nominating these individuals (or organizations)!
Please take a moment to fill out this form with your nomination. All nominations will be kept confidential. If you would like to nominate more than one individual,

NOTE: The HSA committee will make the final decision on the type of the award to be given, but we welcome your suggestion.
Very Special Person Award (VSP) - This award is given to recognize an individual or organization for their service to the PTA.
Honorary Service Award (HSA) – This award is given to recognize an individual for outstanding service to children and youth in our community, going above and beyond what is asked of them.
Continuing Service Award (CSA) – This award is given to recognize an individual for ongoing or long-time service to children and youth, providing support year after year.
Outstanding Teacher Award (OTA) - recognize a teacher for outstanding service in positively impacting the lives and welfare of children in PTA, school, or community.
Outstanding Administrator Award (OAA) - recognize an administrator for outstanding service in positively impacting the lives and welfare of children in PTA, school, or community.
Golden Oak Service Award (GOSA) – This award, the most prestigious of the Honorary Service Awards, is given to recognize an individual that has made significant contributions to the welfare of children and youth in a school or community.
