We are conducting a survey of Program participants and kindly request your participation. The survey seeks to gather information about what knowledge and skills you gained from participation as well as information about your future plans.

Your responses will be kept confidential and will be used only to help us evaluate this year's program and make improvements and modifications to better meet the needs of participants. The survey data will be used to evaluate the effectiveness and value of the Program.
