This the registration form for our high school team camps.  

The High School Summer Camps are offered for 9th through 12th grade student athletes who will be attending Saint Thomas Aquinas High School in the fall of 2023.  Camps are held at Saint Thomas Aquinas unless noted otherwise.  Though not required, the camps are recommended for student athletes participating in high school sports.  

Please complete one form per student per camp.  Camp Payment should be made at the time of registration by making a check payable to the coach.  

By completing this form, you are also approving this Waiver:

I give permission for my son/daughter to participate in the 2023 High School Summer Camp(s)/Conditioning Clinic. I understand that I must provide insurance coverage for my student and acknowledge that Saint Thomas Aquinas High School and any agents involved with the aforementioned summer camp(s) shall be held harmless in the event of illness, injury or loss of equipment related to the program.
