The purpose of this form is to collect anonymous accounts from new students and upperclassmen of (1) uncomfortable situations and (2) rotation violations.

The motivation for this form is that new students in the past had no way of voicing their problems with Rotation situations in an efficient fashion to the IHC. We take each submission very seriously, so please be as accurate as possible in what you report.

If you feel comfortable, please provide a description of the event, the time, duration, location, people involved, and other pertinent information. Keep in mind that events you feel have broken the honor code or simply made you very upset are also appropriate for this form.

Information from this form will be shared with the IHC. These reports are completely confidential.
