Please complete the following Field Trip Volunteer Form if you would like to volunteer for an upcoming GALA Field Trip. ALL Volunteers MUST be LAUSD Volunteer approved to participate and chaperone field trips. Please review the GALA Field Trip Volunteers Sign Up Process flyer, link here.

VOLUNTEER STATUS: If you have any questions regarding your volunteer status, please email, Angie Gomez at President@fogala.org. You will be notified prior to the field trip if you are NOT cleared to volunteer. You can also confirm your status by emailing your Grade Level Lead, list here, if you want to confirm your volunteer status for the 2023-2024 school year. All GALA Volunteers are Tier 2 LAUSD approved for on day field trips and/or on campus volunteer opportunities.

OVERNIGHT Field Trip Volunteers: Overnight Field Trip Volunteers MUST be a Tier 3 Volunteer which requires an extra level of LAUSD approval ie: fingerprinting. In order to start the approval process for Tier 3, you must complete the request for Tier 3 approval. The request will be reviewed and approved by GALA Administration. Once approved you will receive instructions to start the LAUSD Tier 3 approval process. Please review the Tier 3 Volunteer Request section of this form.
