AGES 4-5 (ELIGIBLE FOR KINDERGARTEN THE FOLLOWING YEAR)

Thank you for enrolling your child at Noah's Ark Preschool!  The following information is required for your child's file, so please complete it thoroughly.  The child’s first day of school may be delayed if the required paperwork and/or payments are not received on time!

Required for Registration:
1) This enrollment paperwork
2) $30 enrollment fee + first month's tuition $130 = $160 mailed to Noah's Ark Preschool

What is due prior to the child's first day of school?:
1) Scholarship application (optional)--Due July 1
2) Child's current (within the last year) physical/well-child checkup--Due August 31
3) Child's immunization record or exemption--Due August 31
