Fill out this form if you would like to volunteer for MIRA's in-person Citizenship Clinic on Thursday, February 16th from 9:30am to 12:30pm at the Center for Hope & Healing in Lowell. New volunteers will receive a brief training and orientation at the start, and will then work one-on-one with a citizenship applicant completing the N-400, Application for Naturalization.

By necessity, personal and private information is disclosed in the course of working with clients to complete citizenship applications at the citizenship clinics. Part of what I learn is necessary to provide services to the applicant or client; other information is shared within the development of a helping, trusting relationship. I understand, and agree, that all such information is to be treated confidentially and discussed only within the boundaries of my volunteer position at this organization. I will delete any information from my device after my volunteer commitment ends. *
