This form must be successfully completed for students to be eligible for ISP flexibility. I understand that all grades must be above a C- at mid-term and report card to participate. Furthermore, I understand students who do not attend and fail to meet the requirements will lose Student in Good Standing Status and have further disciplinary action. ISP is 7:30am - 7:55am on Monday, Tuesday, Thursday, and Friday. Any student who has ANY grade below a C- must attend until the grade is raised, mid-term and report card grades dictate.
