Add-On Transcript Review Form
A transcript review request is required as the first step towards admission to the Add-On Program at the Center for Additional Teaching Licenses through UW Oshkosh. This review is necessary in identifying what coursework is needed for the add-on license you are pursuing.

Follow these steps to request a transcript review:
• Complete this form
• Pay the $100 fee with a credit or debit card by paying at https://education-uwosh.nbsstore.net/
• Submit Transcripts for all courses to addon@uwosh.edu. Unofficial copies and grade reports may be accepted at the time of your review; however, be sure to order your OFFICIAL transcripts, as they are required at the time you apply to the Add-On Program.
