I agree to pay a non-refundable registration fee of $35 which is due immediately. Monthly tuition is due on the 5th of each month. Please note if you miss a class you have the opportunity to attend a different scheduled class. Therefore, your monthly tuition is non-refundable. Tuition and the registration fee can be paid through your parent portal, over the phone or check. Tuition not paid by the 8th of each month is subject to a $15 late fee. A card is required to be saved on file but you DO NOT have to use the card for payment.  I understand these polices and agree to them.          *
Studio Wide Spring Recital is April 27th, 2024 at Cross Point Church In Lake Worth. Costume Fee is due January 8th, 2024. Will your dancer be participating?

Uplift Staff will follow-up with this form to confirm your selections.Payment is due to secure your dancer's spot. There is a processing fee of  3.05% + $0.30 per transaction for credit or debit cards. Pay by check to avoid the processing fee. Please sign below with your initials and date.
