Please use this form if you need to remove your swimmer/s from the Cy-Fair Swim Club. All forms must be submitted by the 25th of the month. Forms submitted after the 25th will be processed the following month.

**If you choose to withdraw from the team and wish to return, you will be required to pay a re-registration fee equal to the regular registration fee. Your application to return to the team will go through an approval process to ensure you are in good standing with the team.
