Please fill out this form if you have an approved device to donate.

To ensure we can use your donations, please see our donation guidelines here before submitting the donation form.

Once you have filled out the form, please drop your device off at the KOP office located at Greenovation (22 Flint St- Upstairs, by following the KOP signs) please clearly mark all items so that we can contact you with questions.

IMPORTANT NOTICE: KOP reserves the right to distribute donated technology as we see fit, and to best meet the needs of our clients and the organization. This includes and is not limited to the sale of any items of higher value or beyond what our clients can use. All proceeds benefit KOP services and programing.
