**PLEASE READ ALL OF THE INFORMATION PROVIDED**

This form will act as the official sign-up form for all intramural sports organized by the Carleton Student Engineering Society.

Please select "Yes" if you want a spot on any of the following CSES intramural sports teams: Ultimate Frisbee, Dodgeball, Soccer and Volleyball.

Intramural sign-ups are first-come-first-serve. The first 12 individuals to respond "Yes" for each team will receive a spot on their desired team. By selecting "Yes" you will be given a spot on the team and will be expected to pay the intramural fee of $10 before your second game. Failure to do so will result in the loss of your spot on the team. Payments will only be accepted at the CSES office (Open noon-4 Monday to Friday) once you sign up you will receive a follow-up email from the CSES VP Social.

Intramural leagues start date is yet to be determined.

*DISCLAIMER*
- Priority will be given to CSES members (all students enrolled in a BEng degree) but non-B.Eng students are welcome to sign up.

-as per Carleton University regulations all members participating in intramurals will be expected to have received a full course of WHO approved COVID vaccinations.

-members requesting to be on single teams will be given priority over members already on a team.

-gender is required to ensure that each team is capable of maintaining the gender ratio as specified in the league rules for coed teams
