I hereby request and give permission for my youth to participate in Cheer Camp 2024. I understand and assume the risks inherent with this event and I also understand that all reasonable care and supervision will be exercised to provide for the general well being of my youth. I, on behalf of my youth named above, do hereby release, convent not to sue, and save harmless: The Most Rev Gregory Parks, Bishop of the Dioceses of St. Petersburg, Tampa Catholic High School, and the employees, agents, and volunteers for the event, from any and all claims for any and all harm arising to my youth as a result of their participation in the event. Basic Indemnity Release: In consideration of my opportunity to participate in Tampa Catholic Cheer Team’s sponsored event, Youth Cheer Camp, at Tampa Catholic High School, June 5th-6th, in 2024, I agree to release and on behalf of myself, my heirs, representatives, executors, administrators, and assigns, the Cheer Team, its officers, directors, employees, coaches, volunteers, agents, representatives and insurers, as well as, The Diocese of St. Petersburg and Tampa Catholic High School from any causes of action, claims, or demands of any nature whatsoever including, but in no way limited to, claims of negligence, which I, my heirs, representatives, executors, administrators and assigns may have, now or in the future, against the Cheer Team related to or on account of personal injury or death arising out of or in any way related to the Cheer Team's facilities/equipment or participation in the Cheer Team programs whether that participation is supervised or unsupervised, however the injury or damage occurs, including, but not limited to the negligence of Releasees. Further, in consideration of my opportunity to participate in the Cheer Team activities, I agree to indemnify and hold harmless Releasees from any and all causes of action, claims, demands, losses, or costs of any nature whatsoever arising out of or in any way related to my participation. Release: Coronavirus / COVID-19 Warning & Disclaimer - I understand and agree that Coronavirus, COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact, and that there is an inherent risk of contracting a COVID-19 infection. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Federal and state authorities recommend social distancing as a means to prevent the spread of the virus. Attending public events or participating in large gatherings could increase the risk of contracting COVID-19. I agree and understand that the Tampa Catholic Cheer Team in no way warrants that COVID-19 infection will not occur through participation in the Cheer Team programs or accessing the Cheer Team facilities, including the Cheer Team-sponsored event, Youth Cheer Camp, at Tampa Catholic High School, June 5th-6th, in 2024. Publicity/Photo/Video Release: From time to time, Tampa Catholic Cheer prepares publicity releases for newspapers, television, website, mass electronic announcements, and other social media pertaining to events hosted by Tampa Catholic Cheer. These may or may not be accompanied by photos or videotape of the cheerleading team and/or coaches. The releases may be prepared by Tampa Catholic Cheer and/or their coaches and media representatives. *
