Please submit this form each time you complete volunteer hours.  Please be aware that volunteer hours may be verified.  

New State Distinction Honoring Volunteerism
Students that voluntarily complete at least ten (10) hours of community service each semester the student is in attendance at a public high school shall be recognized at their graduation ceremony. Students are responsible for reporting volunteer hours to earn the Volunteerism Distinction. Volunteer hours must be completed during the semester. Hours completed during summer or winter break will not be counted; however hours completed during fall break, Thanksgiving break, and spring break will be counted.
 
Guidelines for Volunteer Work:
-Student cannot be paid to complete volunteer hours.
-Students may not complete volunteer hours for a family member.
-Proselytizing (sharing faith) may not count, but volunteer work completed with a faith based group may be counted.
