This self-assessment, created by Public Health Madison & Dane County, will help you determine your workplace's readiness to return to in-person work during our county's reopening plan, Forward Dane. Submissions are anonymous and it should take less than 15 minutes to complete. There are a total of 6 sections.

At the end of the assessment, your "score" will be calculated to give you an idea of how your business is doing. The purpose of the score is to help you figure out how you can improve and is not used by PHMDC. Questions are weighted by importance.
