Dear Parents/Guardians,

Please fill out the following information. Any information given will be strictly confidential with only the Band Directors and school Counselor seeing the form once it is turned in. Please complete the form no later than Friday, June 12, 2020. If you have any questions, please feel free to email me at bgarza@ogisd.net.

Please note: The band directors will test all students interested in playing percussion once school starts. Your child must already be signed up for band to be able to audition for the percussion class. We will not promise or guarantee a percussion spot to any student before the testing can take place.
