Use this form to sign up for Rideshare or high occupancy vehicle (HOV). Remember, to be eligible, you must carpool or have three or more of your own children in your car three or more afternoons a week for the entire year.

After you complete this form, please stop by the front office to pick up your rideshare sticker(s) to put on your red driveline tags. Families will not be permitted access to the parking lot during rideshare without a rideshare sticker displayed prominently on their driveline tag. 

The sticker with the current school year on it allows you to enter the parking lot at or before 3:25pm.
