The Mississippi Thunder Kings Hockey Club (MTK) is required to maintain a record of participants for all MTK events, including home games and practices.  Please completed this form within 24 hours of the game or practice.

With respect to games, the home team shall maintain contact tracing records for both the home and visiting team.  The Safety Officer for the MTK team should provide the link to this form to the Safety Officer of the visiting team prior to the event to allow for the visiting team to complete the required form.

The information on this form may be shared with Lanark, Leeds and Grenville Public Health Unit at any time upon request.
