Attendance for rehearsals is essential in ensuring quality performances. However, there are times when a student must be absent, late, or leave early because of an unavoidable conflict.

This form must be submitted at least TWO WEEKS PRIOR to any anticipated absence. Please refer to the band handbook to review the attendance policy.
