Season 12 Competition Team: Audition Form
Please submit this form by May 17, 2024. Pre-registration is required by all dancers in order to audition. A $25 non-refundable audition fee is due at the time of registration. 

Please visit danceprestige.com to register your dancer for our Season 12 Auditions. This form does not count as your dancers registration.

Prestige Dance Center Dancers, Competitors, and or their parents/guardians understand and recognize the risks of physical injury inherent in dance and dance performances and are willing to assume those risks. It is agreed that participants, parents/guardians will not hold Prestige Dance Center, its employees, or independent contractors liable for any injuries sustained or illnesses contracted by them while in any activity or event in PDCs physical location, or in any venue outside of our location. 

Prestige Dance Center will not be held responsible for any transportation by parents, staff, and or students who choose to carpool.
