Mini-Camp 6-12 at Innovation Youth
Join Innovation Youth Library's mini-camp for 6-12 year olds from Wednesday to Friday from 10am-2pm from July 7 to August 18, 2021. Our activities will include gardening and outdoor club, francisation activities, cooking workshops (based on provincial health restrictions), and cultural activities. Registration is required and is open until June 21st, or until spots fill up. There are no fees to join the mini camp, but participants will be required to bring their own lunches.

This form must be filled out by the participants' parent/legal guardian. As part of Innovation Youth's Child Protection Programme, Innovation Youth and its parent organization, Christian Direction, are required to collect certain pieces of information on each participant in order to provide the best care in the unlikely case of an emergency. The information collected will be kept confidential.

I understand that this registration can be terminated if the participants' behaviour is deemed inappropriate or harmful. The staff reserve the right to send the child home if behaviour is such that we don't have the intervention expertise to properly deal with them. *

I, the parent or legal guardian named above, authorize the staff on the premises to sign a consent for medical treatment and to authorize physicians or medical staff to provide assessment and treatment for the participant if the staff were unable to reach the emergency contacts listed above. I, the parent or legal guardian, agree to hold blameless the staff, interns and volunteers against any loss, damage or injury suffered by the participant as a result of being part of the activities of the Innovation Youth. This consent and authorization are only effective when the minor takes part in activities of the Innovation Youth mini-camp. *
COVID-19 RISK RECOGNITION.  This portion of the  Parent Waiver will remain in effect until Christian Direction receives instructions from provincial government authorities and Quebec Public Health, to the effect that the commitments contained in this declaration are no longer necessary. The new coronavirus, COVID-19, has been declared a global pandemic by the World Health Organization (WHO). COVID-19 is extremely contagious and is believed to spread mainly through person-to-person contact. Therefore, government authorities, both local, provincial and federal, recommend various measures and prohibit various behaviours, all with the aim of reducing the spread of the virus.  Christian Direction and Innovation Youth undertake to comply with all the requirements and recommendations of the Public Health of Quebec and other government authorities, and to implement and adopt all measures necessary for this purpose. However Christian Direction cannot guarantee that your child will not be infected with COVID-19 . In addition, your participation in activities could increase your risk of contracting COVID-19, despite all the measures in place.  By signing this document, 1) I recognize the highly contagious nature of COVID-19 and I voluntarily assume the risk that my child could be exposed or infected by the COVID-19 through my participation in Christian Direction activities. Exposure to or infection with COVID-19 can cause injury, illness or other conditions; 2) I declare that my child's participation in the activities of Innovation Youth/Christian Direction is voluntary;  3) I declare that my child, nor any person living under my roof, has manifested symptoms of cold or flu in the past 14 days;  4) If my child (or any person living under my roof) experiences symptoms of cold or flu after the signing of this declaration, they will not participate in Innovation Youth's activities for at least 14 days after the last manifestation of cold or flu symptoms. 5) I declare that neither my child, nor any person living under my roof, has traveled or stopped over outside Canada in the past 14 days. If my child (or any person living under my roof) travels outside Canada after signing this declaration, they will not participate in Innovation Youth's activities for at least 14 days after the day back from trip. *
