Falmouth Public Schools - Superintendent Agreement Waiver Request
Please complete this form to request a Superintendent Transfer Agreement for your child.  

Please note:  
  • Requests for the 24-25 school year will not be processed until May.  Superintendents look at their own enrollments before granting requests.  If we are able to process requests sooner, we will certainly do that, pending enrollment information and communication between districts.
  • Please be advised that a Superintendent's Agreement is subject to approval by both superintendents and that transportation is the responsibility of the parent(s)/guardian(s).
  • This agreement will expire at the end of the requested school year, and there is no guarantee that future transfer requests will be approved.
  • If you have more than one child you are requesting an agreement for, you will need to submit an individual request for each child.
