This form will give your basketball coaches the ability to plan summer activities(camps/shootouts...) around the COVID-19 restrictions.  Please fill this out for each athlete in the home to ensure we get accurate numbers.

What the district will do to keep the students safe: (If activities are held outdoors, there may be different restrictions)

1.  Sanitize facilities after each session. This will require a minimum of 10 minutes.
2.  Doors will be propped open during any/all sessions.
3.  Students and coaches will be required to wear masks. HCR1 will not provide masks.
4.  Students will be required to bring their own drink bottles, towels, braces, wraps, etc.
5.  HCR1 coaches will check the child’s temperature/symptoms daily upon arrival before the student is allowed to participate. Students will not be allowed to participate if they show any symptoms of COVID-19.
     a.  Symptoms include:
               Fever over 100.3F
               Cough
               Sore throat
               Shortness of breath
               Close contact or care of someone with COVID-19.

HCR1 will have a 10-person limit per group until further notice.

Student-athletes will NOT be penalized if they choose not to attend summer conditioning.
