Workplace Accommodation Requests
Thank you for contacting Human Resources (HR) with questions regarding potential workplace accommodations.

In accordance with the Americans with Disabilities Act (ADA), Human Resources will engage in a timely, good-faith interactive process to discuss potential workplace accommodations with qualified staff members who have an impairment that substantially limits one or more major life activities.

The intent of the ADA and the reasonable accommodation interactive process is to identify potential ways in which the district can support staff efforts to successfully perform the essential functions of their jobs and meet district performance expectations.

As permitted by law, you may be required to provide supporting documentation from your health care provider during the interactive process, but we are not asking for medical documentation right now. It is important to understand that any medical information gained during this process will be kept confidential in HR. Only essential information pertaining to your condition, the feasibility of the requested accommodation(s), logistics, and agreements, if any, will be discussed with your supervisor on a need to know basis.

After HR reviews the requested information, we will contact you to confirm your eligibility for workplace accommodations and, if eligible, will - in collaboration with your supervisor - engage in an interactive process with you to determine options for workplace accommodation(s) that are both consistent with district needs and support your efforts to safely and successfully perform the essential functions of your role.

If you have any questions not addressed in this form, please submit an email to hr_ada@4j.lane.edu and someone will get back to you as soon as possible.
