To ensure the maximum amount of magic in this week (July 17-23) full of events, we ask that you please fill out the requested fields below to the best of your ability. This survey will be used to assist the Supreme/Grand Lodges with the planning and coordination of the Supreme Convention. Your survey response will help us to understand who is attending convention, how many people will be attending each event and also allow us to reach out to participants with more information/updates about the Supreme Convention. Please note that completing this survey is not considered registration. Please ensure you complete the official delegate registration form and pay for your delegate package by July 1, available on our website.
