To make this application complete, please include a $150 non-refundable application fee per student.  Your application fee can be paid to Libertas Scholars Leadership (Academic Advisory Board members) at Delaney Baptist on these dates:

>Information Nights (February 24 and March 10)

Please provide the information for each student you are registering for Libertas Scholars.  Every student in grades 3-12 will need a UNIQUE email address for the student assignment / grade portal (cannot be the same as siblings or parents).  If your student does not currently have an email address, please set one up for them to use during the school year.  Many of our families have used NameLibertas@gmail.com so they only receive Libertas related email in this account.  If your child has a common name, you may want to include the initial of your last name.

Please list any information regarding the learning or behavior of your child(ren) that you feel would be appropriate for your child(ren)’s tutor to know.  We want to partner with families to meet the needs of their children and this allows us to be sensitive to their needs from the beginning.  *If your student(s) have an IEP (Individualized Learning Plan), 504 Plan or any other accommodation from the public school system or require additional learning support (such as ABA, specialized tutoring, etc.) we ask that you please share this as well.  We have an Inclusion Specialist who will meet with you to discuss how we can best meet the needs of your student at Libertas.   *
Libertas Scholars is a homeschool community and partnership between parents, who are the primary teachers, and tutors.  The success of our program is helped by parents who regularly and enthusiastically serve as volunteers.  Do you agree to volunteer at in accordance to our policy manual? What skills/talents do you have that could benefit the Libertas community?  How would you prefer to volunteer?  A few key roles include lunchtime gym monitor, study hall monitor, campus parent of the day (assist with logistics and fill in as needed throughout campus), substitute tutor, other....   *
If applicable, please list any other adults that you would approve to accompany your child to community day or serve in a volunteer capacity if you were unable.  (examples: grandparents, aunts/uncles, neighbor, etc.)  The adults you list will also be able to drop off and pick up your child(ren).  Please include their full name, relationship and a contact phone number. *
Does your family have any activities which we should be mindful of?  (regular sports, drama, music, debate, family camping trips, movie nights, etc.?)  Remember, you are still your child(ren)’s teacher and ultimate authority.  We ask that you partner with your tutor in advance if scaling assignments must occur due to family obligations.  Missing class is acceptable when notified in advance, but parents may then be responsible for missed material and grading. *
How much time for homework is tolerable in your family? *
Libertas Scholars has a dress code: solid color polo or t-shirt or Libertas spirit shirt with pants, shorts, skirts, or capris that are at or below the knee.  The specific guidelines can be found in our Policy Manual.  Student(s) will be asked to sign a Conduct Covenant Agreement that will cover this topic prior to starting classes.  Will you ensure your student(s) adhere to this while on campus? *
Libertas Scholars has a nonuse cell phone policy for students during the hours of 8:00am and 3:45pm (with the exception of the lunch hour.) The specific guidelines can be found in our Policy Manual.  Student(s) will be asked to sign a Conduct Covenant Agreement that will cover this topic prior to starting classes.  Will you ensure your student(s) adhere to this while on campus? *
