By initialing below, you agree to the following terms:

1) You permit your child to be tutored by a high school student at 425 Tutoring

2) You may have to sign off and verify volunteer hours for your tutor at the end of the sessions

3) Your child will attend meetings that are scheduled and agreed upon by both parties, as tutors are volunteering their own time

4) You will give your tutor a 24 hour notice if your child is unable to make a session

5) In the event where four or more sessions are missed, you understand that you may be reassigned to a different tutor/time or asked to re-enroll when sessions are a better fit for your schedule
