⭕上色費用須一次繳清。遮瑕費用經評估後可轉帳。若不確定金額歡迎選手和我們先聯絡查證 （聯絡資訊於表單結尾）。跨組者均收一次上色費用並享無限次服務。
⭕臨時無法參賽選手可將預約轉讓，但不得退費。轉讓需選手自行處理後通知LSR。
⭕One time payment to enjoy unlimited service with no extra cost for crossovers. 
⭕No refunds once payment is complete.

⭕ LSR為本賽事官方膚色服務，上色地點為新莊宏匯廣場11樓。 Tanning service will be on the 11th floor inside the venue. 

⭕ 完成此表單並付款成功後7日內將發送預約成功信件至Email。Confirmation email will be sent within 7 days of completing this form and payment.

■ 上色收費價格
[定價 Regular] 3,500NT *報名至6/1- 12:00截止，registration ends June 1st 12:00
⭕️上色名額有限，欲報名上色請盡快報名，額滿後報名表單將自動關閉，6/8現場也無法受理報名。
⭕️ Few spots remaining, once we are full, we will not accept anymore bookings or walk ins.

⭕現場價限6/8過磅當天16:00以前，名額有限。
⭕Walk in only available at on June 8th 16:00, (subject to availability).
⭕️名額有限，6/9比賽當日不收現場報名! 現場報名者將排到最後梯次，等候時間需是狀況，請儘早線上報名。
⭕️Limited spots only. No walk ins accepted on the 9th! Walk ins will be queued last in line for tanning, wait times are undetermined.

■ 上色服務範圍
6/8: 第一層膚色 First coat
6/9: 第二層膚色 Stage coat
⭕比賽當日補色(無限次)、亮油(無限次)、服裝固定(無限次)、刺青遮瑕 (需加購)，妝髮(需加購)。
⭕Includes touch ups, glazing, and suit adjustments. You can purchase unlimited tattoo/scar coverage & hair/makeup service.
⭕以上操作皆由LSR工作人員在指定位置為選手服務。
⭕All procedures will be carried out by LSR staff at designated areas. Kindly follow us on IG @lsr.tw

報名注意事項
⑴ 為確保服務品質，服務人數有限。若超過名額者將通知辦理退費。
     To preserve service quality, we strictly avoid overbooking. Please make sure to register on time.

⑵ 報名後7日內將依付款順序發出確認信才算報名成功。
     We will send booking confirmation via email within 7 days of receiving payment.

⑶ 請留下正確的e-mail，報名成功與否將由e-mail通知。若有急事才會用電話聯絡。
     Please provide accurate contact info, all booking infos will be ONLY via email, we will call only for urgent situations.

⑷ 選手「上色的注意事項」於報名成功後發送至e-mail。
     Upon confirmation, you will receive guidelines to follow before your tanning appointment. Please read it carefully.

⑸ 請選手務必於報名成功後詳讀信件內容，感謝配合。
     Kindly read all our email notifications thoroughly to avoid any issues.

⑹ 本次服務費用一律不退費，請確認後報名。
     Our services are non-refundable.

1. 上色: 本人無傳染病、皮膚病、氣喘等疾病並瞭解當本人接受本服務時，可能導致包括但不限於暫時性仿曬效果、皮膚癢，皮膚乾燥或呼吸不順，如遇到上述狀況與LSR台灣及其工作人員無關。LSR團隊均通過 Liquid Sun Rayz官方上色服務訓練。＊ 如有上述情況，請先與LSR工作人員聯絡，讓我們為您想辦法＊

1. Spray Tan
I hereby confirm that I do not have any transmittable illnesses including but not limited to, skin diseases and respiratory illnesses. I fully understand that spray tanning may cause skin dryness or irritation, as well as possible breathing difficulties. If in the event that I encounter the aforementioned conditions, I vow to waive LSR Taiwan and its staff from any liability. LSR technicians are fully trained to provide tanning and/or hair, make up services.  *Please let us know if you have above conditions so that we may assist you!

2. Hair & Makeup
All brushes and makeup products are kept sanitary and are sanitized between every makeup application. Makeup products used are hypoallergenic.

Client acknowledges that he/she has or will consult their physician or dermatologist prior to receiving makeup services; will report any skin condition to the makeup artist prior to application, and, if needed, request a sample test of makeup to be performed on the skin to test for any potential adverse reaction prior to a full application of makeup products.

If Client has any adverse reactions to any makeup products, Client expressly agrees to seek immediate medical treatment for such adverse reaction.

Client further agrees to release the makeup artist from any liability from all skin complications due to allergic reactions.
