I understand that photograph(s), video(s), interview(s), other image(s), and/or audio recording(s) may be taken of my child by agents, employees or representatives of the Fresno Madera Counties Music Educators Association in order to market or disseminate information in its academic and public service programs to the general public.

I hereby irrevocably authorize FMCMEA and its agents, employees, or representatives to copy, exhibit, publish, or distribute any and all photographs, videos, interviews, and/or audio recordings or other images of my child on in which they appear, including composite or artistic representations, and to us said photographs in all forms and media for purposes of publicizing the organization's program, or for any other lawful purpose. In addition, I waive any right to video(s), interview(s), and/or audio recording(s) appear.

I hereby hold harmless and release and forever discharge FMCMEA and all its agents, employees, or representatives, and their successors, from all claims, demands and cause of action which I, my heirs, representatives, executors, administrators or any other people acting on my behalf or child's behalf or my estate have or may have by any reason of the authorization.
