Wedding Client Intake Form
We are thrilled to take your pet to your wedding! Please fill out the following form to the best of your ability.

We cannot accept requests around major holidays such as Thanksgiving or Christmas.

We only accept wedding requests within 90 days of the event. We understand that you need to plan in advance however we cannot guarantee staff availability more than three months out.

Start and end times (example 4 pm pickup - 8 pm drop-off) *
Itinerary - Please describe the pet's itinerary and include times and locations. For example, pet arrives at venue 3:30 pm. Wedding planner greets handler. 4:00 First Look - bring pet to holding room downstairs. 4:15 - 4:30 photos - bring pet to courtyard,  4:30-5:00 bring pet outside to walk during downtime, 5:00 line-up for ceremony, 5:15 pass pet to bridesmaid, 5:20 pet walks down the aisle then bridesmaid passes pet to handler, 5:20-5:45 pet in holding room during ceremony, 5:45 - 6:00 photos bring pet in front of the venue, 6:00 - 6:30    bring pet to first part of reception next door. 6:30-7:00 take pet home, walk and feed. *
Supplies: Where will the supplies be kept? Leash, water bowl, outfit, paw wipes, brush, toys, treats etc. *
Wedding Planner/Day of Contact: *
Behavior: Is the pet trained with basic manners? Does the pet have any triggers - example avoid kids, avoid dogs?
Has a walker or sitter ever come in to your home when your pet was alone?  If yes, was the pet aggressive, resistant, or fearful?

Pickup location of dog for transport (usually owner's home) - Please provide the full address *
The first location dog will be driven to (usually the ceremony venue or reception hall, sometimes bridal suite at a hotel etc.) - Please provide the full address *
The second location dog will be driven to (the venue, reception, photos etc.) - Please provide the full address (if applicable)
At the end of the event the dog should be taken to (usually owner's home or hotel suite) - Please provide the full address (if applicable)
Consultation: This usually takes place at your home and can take 15 - 30 minutes. Please suggest a few dates and times when you and your pet are available for a consultation.  Ex. Saturday, May 10 from 10 am - 4 pm or Sunday, May 18 12:00 - 3:00 pm. *
