If you will be picking up your student(s) at the end of the school day, please complete this form. Dismissal placards will be sent home in our take home folders during the first day of school.  Each family will receive 2 copies per a student. If additional cards are needed, please indicate in the survey below. Also, please come prepared with a driver's license or alternate identification for the first day of pick up.
