Dear Parents and/or Guardians,

Urban Impact Ministries would like to invite your child to our After School Academic Program (ASAP).

We meet on Mondays, Tuesdays, and Thursdays from 4pm – 6pm.

Your child will either be dropped off at Urban Impact around 4 o’clock (if they don’t go to KIPP) or get picked up from KIPP Central City Primary (KCCP) between 3:50 and 4 pm on Mondays, Tuesdays, and Thursdays. Our tutors will work with your child on their homework assignments, projects, and other educational activities.

All three days your child will be given free snacks. Once after school ends on Tuesdays, your child will either be dismissed to SLAM, which is our middle school youth group for 10- to 13-year-olds, and given a ride home at 8:00pm, or given a ride home at 6:00pm. While on Mondays and Thursdays, he/she will be given a ride home at 6:00pm.

To be able to better help your child at ASAP, we are asking that you give permission for their school to release standardized test scores and/or information from their IEP’s (if your child has one) to use at UIM. Please fill out the form below. Please specify if your child will need a ride home at 6 or if they will stay for SLAM until 8 on Tuesdays. No child will be allowed to walk home.
