Elementary Clubs Application Form
Dear HSEnd Hawks,

After-school clubs are here for the Spring Semester! Students may sign up for one club per day based on their grade level. Clubs are not offered on Fridays. Clubs will be held after school until 5:00 pm Advisors will inform students about materials if needed any. Please select the club of your choice for the Spring Semester. Club enrollment spots are filled on a "first-come, first-serve" basis.

By typing in your name, you, the parent, are signing this after-school club form. You also understand that this form does not mean that your child will be automatically placed after school club. You hereby give consent to have your child placed for the after-school club program. Once you fill out this form and choose the club, the payment ($50 per club) should be made on RevTrack and bring us the receipt. There are no refunds. Please fill out the fee waiver form if you are in financial hardship. *
A copy of your responses will be emailed to the address you provided.
