동양선교교회 현장예배 안전수칙 OMC On-site Service Safety Policy

나는 다음의 사항들이 모두 사실임을 진술하고 확인합니다. I represent and confirm that the following statements are true.

(1) 나와 나의 가족, 또는 동거인은 최근 14일 동안 기침, 숨가쁨, 호흡곤란, 발열, 오한, 오한으로 인한 경련, 근육통, 두통, 목 부음, 또는 입맛이나 냄새 감각 마비 등과 같은 질병의 증상들을 경험하고 있지 않습니다.  
I, nor members of my household, have not experienced any of the symptoms illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell within the last 14 days.

(2) 나와 나의 가족, 또는 동거인은 14일 이내 코로나 확진자나 감염의심자에 노출되지 않았습니다.
I, nor members of my household, have not been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 within the last 14 days.

*성도님들의 안전과 지역사회의 바이러스 감염 방지를 위해 위 사항에 해당되시는 분들은 반드시 자가 격리를 하시고 가정에서 예배를 드리시기 바랍니다.
For the safety of our church members and prevention of the spread of COVID-19 in our community, if any of the above items for Coronavirus applies to you, please quarantine yourself and worship online at home.
