The Chapin School looks forward to hosting the DAIS event on Friday, December 9, 2022, from 6:00 - 8:30 pm!

1. ADVANCE REGISTRATION IS REQUIRED for ALL attendees.

2. All attendees must bring proof of Covid vaccination AND official photo ID (current school ID or NYS ID) the day of the event in order to be admitted.

3. The ID must match the name used to register.

4. Once you've been checked in, leaving and re-entry will not be allowed.

PLEASE NOTE: This registration form will close on Thursday, December 8 at 3:00 pm. Onsite registration will NOT be available the day of the event.
