This is a confidential survey to help us commemorate workplace wellness week by auditing the lived experiences of employees and creating a snapshot of the situational analysis in order to create a better workplace wellness policy framework. The intended purpose of this independent survey is to give employers an informed framework that helps them make the best policy decisions, the information entered herein cannot be utilized for purposes beyond that, nor in employer-employee legal proceedings.
