A roster showing name, address, and e-mail of participants will be available at the weekend. May we include your information?

NOTE: Please do not choose “Either” unless you are comfortable dancing both roles and can easily switch roles in order to help role-balance the room.

We provide generous snacks during breaks throughout the weekend, and participants are encouraged to stay on-site during the Saturday lunch break.  For your convenience, we will provide an optional catered lunch at an additional cost of $10 per person. (See PAYMENT information below.)

Do you want us to try to find a place for you to stay with a local family? A limited number of home stays are available. You should have a backup plan, such as a hotel reservation, in case we cannot find a place for you. All requests for home stays must be received with this registration form. If you are not requesting a home stay, you can safely jump down to the Liability Waiver. If you are requesting a home stay, please answer all of the housing questions below.

Most homestays will NOT be within easy walking distance of the Folk Center. Please check all that apply.

Is there someone with whom you would like to share housing? If so, please enter their name below and state sleeping preferences such as desire shared bed, need separate beds but can share room, need separate bedrooms, etc.

Do you have any other restrictions (such as gender of host, non-smoking only, allergies, or space requirements)?

I understand and agree that there are risks of illness and injury associated with, arising out of, and inherent to the activity of dance. Possible health risks include, but are not limited to exposure to respiratory illnesses such as COVID-19 and influenza. Possible sources of physical injuries include, but are not limited to, missteps, dizziness, loss of balance, slipping, falling, and colliding with other dancers or objects. In recognition of this, I agree to assume all risks and responsibility for any such illnesses or injury, which which may accrue to me during the dance weekend sponsored by Lexington Vintage Dance (LVD) February 10-12, 2023. Furthermore, I do hereby fully exempt, release and indemnify LVD, its officers, volunteers, and/or instructors and the City of Berea from any and all claims from illnesses, injuries, damage or loss which I may have or which may accrue to me as a result of my participation in the dance weekend. I also give LVD permission to use any picture of me at the weekend in or on any form of publicity for LVD or an LVD-sponsored event. Registrations are for one person only. Please fill out a form per person if registering multiple people. If you would like to make a tax deductible donation to LVD, please use the other line and indicate your registration type (student or non-student), whether lunch is desired, and the amount of your donation. If you are requesting  the student rate, tell us where you are enrolled.

We will notify you by email when your registration is tentatively accepted. For online payments, we use an online service provider called Stripe which uses most stringent level of certification available in the online payments industry. You may pay with almost any credit or debit card or electronic funds transfer from your bank. Alternatively, you may simply mail us a check via the US Postal Service. With any of these choices, we offer a 7 day grace period for you to pay after receiving notification of your tentative acceptance to the weekend.  Hence, your particular payment choice will not alter your acceptance status.

Since combining the payments of registration partners is fine, we need to know the name of the expected payer if it is not the same as the registrant's badge name.. Your registration will be fully confirmed once payment is received.
