To receive a refund on your student's MySchoolBucks lunch account, please complete this form. Refund checks will be mailed out to the address that you provide within 4-6 weeks of submitting the form.
Complete one form for each student in your household.
* Please note - second meals and all a la carte items - including milk purchased individually - are paid items that either require funds in your student's MySchoolBucks account or to be paid for with cash. Students may not incur a negative balance in their school lunch account.
