2022-2023 School of Excellence - Baseline Survey Questions
The NW PTA is committed to mental health advocacy & drug awareness and are fully supportive of the mental and emotional health of our students and youth. We are asking students to complete this survey to help us, help you address drug awareness, and the mental & emotional health needs and concerns. Filling in this survey will help us provide and improve services that are available for our students. ALL ANSWERS WILL REMAIN ANONYMOUS. Please answer this to the best of your ability in relation to your youth and the youth in your lives.
