Six Flags Parental Consent Form
PARENTS, PLEASE KEEP IN MIND THAT: Six Flags Magic Mountain is a high thriller theme park with intense rides. If your child doesn't like these kind of rides, maybe this event is not for them.

All kids attending the Six Flags trip on Sunday, May 25, 2025 must have this Parental Consent Form completed and submitted on or before Saturday, May 24, 2025.

This consent form is for the trip to:
Six Flags Magic Mountain
26101 Magic Mountain Pkwy
Valencia, CA 91355

Leaving Church - 8:15 a.m.
Arriving at Six Flags - 9:00 a.m.
Leaving Six Flags - 9:30 p.m.
Back at Church - 10:30 p.m.

Checklist on what to bring:
1 - Printed purchased park ticket online (Link to purchase on T4C web page)
2 - Dress accordingly to weather
3 - Money for food and snacks *See note below
4 - Cell phones (optional)
5 - Small backpack to keep personal belongings
6 - Sweater or Jacket (According to weather)
7 - Sunscreen

Note: Six Flags DOES NOT ACCEPT CASH. All purchases are by card only. Parents can Zelle the amount you want your kid to have and we will buy the things for them, or give them a card for purchases.

Food and Drinks
No outside food is allowed in the park. Food and Drinks can be added to your purchase online.

Tickets
Click on the link provided on VallejoDrive.Church/T4C page to purchase the Daily or Season Pass. Note that Season pass will allow them to enter Hurricane Harbor this summer.

Lost Items
* Though we will try our best to retrieve it, we are not responsible for lost items. e.g.: Cell phones, sun glasses, backpacks, or others.
