There are three parts to this registration process. Please complete all three in order to ensure your enrolment in the program.

1. REGISTRATION FORM

Which program are you joining? (if you have multiple children, please complete a separate form for each child) *

ONLINE Program A (ages 8 - 16, Wednesdays, 12pm to 1pm), $50/month

LIVE Mini Program (ages 6 - 7, Mondays 10am - 12noon), $85/month

LIVE Junior Program (ages 8 - 12, Mondays 10am - 3pm), $110/month

LIVE Intermediate Program (ages 11 - 15, Mon & Fri, 10am - 3pm), $210/month

LIVE Senior Program (ages 13 - 21, Thu & Fri, 10am - 3pm), $210/month

2. AGREEMENTS

Photo and Film Release

I agree to have photos or videos taken that include my minor child and that may be used for promotional purposes

Parent Participation

I understand that parents might be asked to volunteer before or during shows

I understand that parents of younger children may be required to remain on-site to support a smooth transition into the program

Liability

The TES Theatre Company staff and volunteers cannot be held liable for personal injury or loss, theft, or damage of personal property

3. PAYMENT

Payment can be made in two ways:

You can pay the year's total now, all at once.
OR you can pay equal monthly payments, beginning now, and then on the first of each month up until (and including) June.

Online: $50 month
Live Mini: $85 month
Live Junior: $110 month
Live Intermediate: $210 month
Live Senior: $210 month

NOTE: If you plan to use some DL allotment funds, contact us at testheatrecompany@gmail.com and we will invoice your DL.
