The following form is to be filled out prior to your child's start of drop in care. If you have multiple children, you only need to fill this out once. Skip the spaces for other children if you do not have more children. These responses will be printed and placed in your child's file, so please make sure all the information is accurate and up-to-date. Some information may be repeated, and we apologize. These forms and questions adhere to being a licensed day-camp with the State of Wisconsin. YOU WILL NEED YOUR CHILD'S IMMUNIZATION RECORDS and a copy of the Health History and Emergency Plan form (the link can be found in the original email sent with this form link.) You will be prompted to e-mail them at the end of this form. Your child is unable to attend camp without the completion of these forms.

I. General Information - Policy Effective: May 1, 2022
Gymfinity Children’s Activity Center is licensed by the State of Wisconsin, Department of Children and Families (www.dcf.wisconsin.gov). Gymfinity is licensed to care for no more than 50 children at any one time. Gymfinity is inspected regularly to ensure that it meets licensing standards.
Child care services are available without discrimination on the basis of sex, race, color, creed, disability, sexual orientation, national origin, or ancestry.
Gymfinity Children’s Activity Center will provide care for children ages 5 through 12 years, who are independently potty-trained.

Child care services will be provided between the hours of 7:45 a.m. and 3:30 p.m., Monday through Friday. No service will be provided on Monday, July 4th, 2024 through Friday, July 8th, 2022. If a holiday falls on a Saturday, we will be closed the previous Friday. If a holiday falls on a Sunday, we will be closed the following Monday.

Gymfinity posts the following items for your review:
Any stipulation, condition, exemption or exception that affects the license results of the latest monitoring visit (Noncompliance Statement and Correction Plan or Compliance Statement).
Any enforcement action, order, forfeiture, temporary suspension, denial or revocation issued by the Department as soon as it is received. These items will remain posted until the violations have been verified as corrected and the action is closed.

Gymfinity Children’s Activity Center’s procedure to ensure that the number, names, and whereabouts of children in care are known to the provider at all times includes having the camp director maintain a roster with parental contact information and other important numbers in case of emergency at all times, taking attendance at the beginning of the camp day, doing an hourly head-count of the children during activities on or off the premises, taking attendance during an emergency (ie: fire, weather related emergency, etc) to ensure all kids are accounted for, taking attendance upon arriving to a field trip off the premises, and taking attendance again prior to departing from the field trip location.

Parents will receive a pamphlet, “Your Guide to Regulated Child Care” which is a summary of child care licensing regulations, as part of an enrollment packet.

Parents are welcome to visit Gymfinity’s camp program at any time during the hours of operation with an appointment to ensure a staff person is available to assist and answer any questions, unless parental access is prohibited or restricted by a court order. If so, Gymfinity will need a copy of the order. Please understand that Gymfinity cannot legally limit access to a parent if there is not a copy of a court order on file at the center.
Gymfinity is required to maintain a current, accurate written record of daily attendance for all the children. Please assist us in meeting this requirement by signing your child(ren) in and out of the center on the required Daily Attendance Record.

Children will only be released to persons listed on the enrollment form. If anyone other than the child’s parent or someone who is listed on the enrollment form is to pick up a child, Gymfinity will need to be notified in writing or by a telephone call in advance. The person picking up the child may be required to show a driver’s license or other picture ID.

If parents wish to allow a school-age child to leave or arrive at the center unescorted, they must provide written authorization for this activity. School-age children who leave the center unescorted must be traveling to home, school or another activity where adult supervision is present. Parents may use the form “Alternate Arrival/Release Agreement – Child Care Centers” to provide this information to the center. We do advise and recommend a parent or guardian pick up the child from the premises.

If the parent or other authorized person arrives to pick up a child and that person appears to be intoxicated or under the influence of drugs, all reasonable steps will be taken to prevent the person from leaving with the child, including offering to call a cab or another contact person.  While Gymfinity cannot legally withhold a child from the legal guardian; we will not hesitate to call the local authorities if we feel the child is in danger.

It is important that we communicate daily concerning the needs and interests of your child.  If there are issues or concerns that need to be discussed, please work with Gymfinity to arrange a convenient time to talk on the phone or in person so we can give the issue the attention it deserves. To foster communication on a regular basis Gymfinity Children’s Activity Center provides scheduled e-mailed newsletters.

To protect each family’s confidentiality, Gymfinity Children’s Activity Center will not disclose personal information regarding a child or facts learned about a child or child’s family to anyone who is not authorized to receive this information. Persons that have access to children’s records will not discuss or disclose personal information regarding the children or facts learned about the children and their relatives except to the parent or other persons authorized by state law.

Liability Insurance on Premises:
Gymfinity Children’s Activity Center is covered by liability insurance for the premises.
Liability Insurance on Business Operations:
Gymfinity Children’s Activity Center is covered by liability insurance for the business operations.

All child care providers are mandated reporters of suspected child abuse or neglect.  If a child care provider suspects a child has been abused or neglected, that provider is required to report the abuse or neglect to Julie Ahnen, Child Protective Services Manager at the Dane County Department of Human Services at 608-261-5437. Each child care provider and substitute will receive training at least every 2 years in child abuse and neglect laws; how to identify children who have been abused or neglected; and the procedure for ensuring that all known or suspected cases of child abuse or neglect are immediately reported to the proper authorities.
 
Wisconsin’s Concealed Carry Law:
Wisconsin Act 35 relating to carrying concealed weapons went into effect November 1, 2011.
No person may carry weapons while on the premises of the child care center during licensed hours.  Signs (at least 5” x 7”) providing notice of refusal to allow weapons are posted near all probable access points where any individual entering the building or grounds can be reasonably expected to see the sign.

Gymfinity's Bummer Free Summer Camp participants are REQUIRED to provide immunization records of their vaccination against COVID-19.

II. Enrollment and discharge of enrolled children: Policy Effective: May 1, 2022
If Gymfinity must remove a child from care within the facility due to reasons listed further down in this section, Gymfinity will issue a full refund for any remaining full days or weeks that were paid for to attend camp.
A parent can terminate their child at any time; however,  the Gymfinity Summer Camp refund policy, regardless of reason, will be enforced.
Parents must meet with the Camp Administrator to discuss their child’s specific needs and to review program policies if the parent deems it necessary. Gymfinity will make a reasonable accommodation for a child with disabilities as specified under the Americans with Disabilities Act.

The following items must be completed and returned to the center by the first day of attendance.

Gymfinity will inform the parent/guardian of any updates that are needed and give 2 days to submit the updated forms.

The following items must be completed and returned to Gymfinity upon paying the registration fees for camp.

Gymfinity does not accept children for drop-in care. All children attending camp must be pre-registered through the Gymfinity website or by contacting the Gymfinity office.
A child may be discharged at any time from the center for reasons such as, but not limited to:

-Lack of parental cooperation.
-Inability of the child care program to meet the needs of the child.  The Camp Administrator will consult with the parent to solve concerns or problems before ending the care arrangement.  The parent will be referred to other community resources.
-Repeated failure to pick up the child at the scheduled time.
-Failure to complete and return required forms.
-Failure to comply with the terms of the child care contract.
-A child that is not independently potty-trained and has had multiple accidents during camp.
-The child putting themselves or others (staff or other children) at risk for injury
-Physically harming other children or staff; hitting, biting, punching, etc.
-Bringing a weapon to camp
-Repeated incidents not following direction and listening to the requests of the counselors.

Gymfinity will document, using a specific Gymfinity created disciplinary form, each time a concern about a child arises, and it will be discussed with the parent. After talking with the parent(s), the document will be signed by the parent and will be placed in the child’s file to acknowledge the conversation was held. After three documented instances of having to remind the same parent of a policy, his/her child’s behavior, etc.; the child will be removed from camp.
At the end of a camp day, when a parent signs out their child, there will be a box next to their name that notates if they need to meet and discuss their child with a coach, director, or administrator. If the parent does not talk with the staff person before leaving, it will be notated in their file. It will count as lack of parental cooperation and may result in removal from camp.

Gymfinity will give a day’s written notice of its intent to discharge a child, and try to inform the parents of other local resources that may be of help to them, except when the discharge is due to a parent’s failure to meet the reasons listed above. A day’s notice is appropriate due to the parent being aware of each time a counselor or the director has addressed them regarding a concern with the documentation paperwork. During the second conversation, the parent or guardian will be specifically informed and the interaction will be written down. The third time will be grounds for dismissal from the camp. Parents will receive verbal notice of dismissal and a written notice via e-mail.
Gymfinity does not offer make ups, credits, or refunds for missed days or weeks of camp. If we are notified of your drop from enrollment 30 days or more before camp start, you will receive a full refund of tuition minus the deposit made at registration. If we are notified of your drop from enrollment within 11-30 days of camp start, you will receive 1/2 of your tuition minus the deposit made at registration. If we are notified of your drop from enrollment within 10 days of camp start, you will receive no refund of tuition or the deposit made.

III. Fee payments and refunds: Policy Effective: May 1, 2022
At the time of registration, your deposit is due. For those enrolled in single days, you will be charged 1 day per month enrolled as a deposit. For those enrolled in full weeks, you will be charged 1 week per month enrolled as a deposit. For those enrolled in the entire summer, you will be charged 1/3 of the summer's full cost as a deposit. The remaining weeks will be billed monthly using an auto billing system on the 21st of the month. Gymfinity recommends that all families have valid, credit-card information on file.

Cancellations: If we are notified of your drop from enrollment 30 days or more before camp start (Monday, June 13), you will receive a full refund of tuition minus the deposit made at registration. If we are notified of your drop from enrollment within 11-30 days of camp start, you will receive 1/2 of your tuition minus the deposit made at registration. If we are notified of your drop from enrollment within 10 days of camp start, you will receive no refund of tuition or the deposit made.

Late fees: At the start of business on the 2nd, a $25 late fee will be added to unpaid accounts.  All credit card information must be current to avoid a late fee.

If there will be a third party payment, from an employer or the county, a special payment schedule will be arranged and detailed in the contract. Parents will be responsible for any specified co-payments or unpaid amounts.

The policy on third party payments is as follows:
 
A third-party payment contract must be signed acknowledging that the third party is paying for camp. The third party is able to decide if they will pay any additional fees, should they arise; late pick-up or late payment fees. Third party payments can be paid with a check, either mailed or dropped off. Payments should be received by Gymfinity on or before the 1st of the month for that month of tuition to avoid the $25 late fee as stated above.
 
Registration/enrollment fees are as follows:

Gymfinity's Family Discounts apply to summer camp:
10% off for second enrolled sibling
20% off for third enrolled sibling
50% off for fourth enrolled sibling

If you are late to pick up your child, there is a $25 fee that will be charged to your account on the day the late pick up occurs. Gymfinity will not tolerate late pick ups and families that consistently violate this rule may be asked to leave camp without refund of tuition.

IV. Child and Provider Absences: Policy Effective: May 1, 2022
If your child will not attend on a day you have registered and paid for, please let Gymfinity know by
7:00 p.m. the day prior so it can be notated on the attendance report. We do not offer refunds or make-ups for cancellations or missed days for any reason, including illness or injury unless a doctor’s note is provided for the injury or illness. COVID-19 quarantine is an exception.

On the day of a Field Trip, Gymfinity will not hold the bus and wait for anyone arriving late to camp. Please be on time. If you arrive late and the bus has already left, you will not be refunded and unfortunately, your child will need alternative care for that day as there will not be staff available to provide services after the transportation has departed.  You are welcome to bring the child to the field trip as long as specified plans and communication takes place with the staff on the field trip, the Camp Administrator, and the office staff.

Provider absence:
Vacation: During the summer camp months, we have one week of vacation. For 5 days, Monday, July 4th, 2022 through Friday, July 8th, 2022. Camp will resume the following week.

Illness: Gymfinity will run camp every day, regardless of any staff illness because there will be qualified person(s) to carry out the expected activities and field trips.

Unplanned absence: In case of an emergency situation that requires immediate attention, Gymfinity will call J. Orkowski, Gymfinity’s owner.  Parents will not be called to pick up their children as the backup will be able to fulfill any and all duties required. Each time an emergency situation occurs, Gymfinity will provide the emergency back-up person with a brief orientation immediately before carrying out the shift.  The orientation will include all of the following:

The names and ages of children present.
Arrival and departure information for each child including the names of people authorized to pick up the child.
The location of the children’s files including emergency contact information, consent for emergency medical treatment and any special health care needs.
Regular fees WILL NOT be charged when Gymfinity is closed due to an emergency, and families will receive a refund for the day. If an emergency occurs, families will be informed about the emergency and advised that they will receive a refund.

Planned absence:
In the event that Gymfinity will be closed, parents/guardians will be given at least 24 hours advance notice of the scheduled closure.

If there is a replacement camp director for that day, the substitute will have had an orientation which has been completed and documented on a form provided by the Department of Children and Families and placed in the employee’s file. The orientation will include:
Names and ages of all the children in care.
Current arrival and departure information for each child including the names of people authorized to pick up the child.
Review of children’s records including emergency contact information.
Specific information relating to child’s special health care needs including medications, disabilities or special health conditions.

Overview of the daily schedule including meals, snacks, nap and any information related to eating and sleep schedules of infants and toddlers.

Review of center’s procedures for dealing with emergencies.
Procedure for reporting suspected abuse and neglect of a child.
Plan for evacuating sleeping children if night care is provided.
Procedure to contact a parent if a child is absent from the center without prior notification from the parent.
Review of center policies required under 250.04(2)(e).
Review of chapter DCF 250 Family Child Care Centers.
Review of DHS 12.07(1) regarding caregiver reporting requirements.

V. Health: Policy Effective: May 1, 2022
Each child in Gymfinity’s care will have a file with their health needs clearly noted. Counselors and Directors are required to review each child’s needs prior to the start of the day to ensure they understand a child’s specific needs.

Gymfinity will have an on-site health supervisor should anything arise. This person is certified to handle illness, injury, etc. and will assess the situation; calling a parent if necessary or calling 9-1-1.

a. Child Illness / Injuries
Children who are ill are not to be brought to the center.  Illness will be defined as:
A temperature of 100 degrees Fahrenheit or higher
Vomiting or diarrhea has occurred more than once in the past 24 hours
A contagious or potentially contagious ailment or illness such as the common cold, lice, chicken pox, strep throat, pink eye or a wound that may reopen, etc.
An unidentified rash
Has not been on a prescribed medication for at least 24 hours or continues to have symptoms of illness
Has a constant, thick, colored nasal discharge
A child has pain indicated upon normal movement

If a child should become ill while at Gymfinity, parents will be contacted immediately.  Sick children will be isolated within sight of the camp director or within hearing distance and made as comfortable as possible.  Children should be picked up within the hour, once a parent has been called.  If the child is not picked up within the hour, the emergency contact person on the child's enrollment form will be called.

Children may return to the center when they are symptom free or have been appropriately treated and have been given written approval to return to Gymfinity by a medical professional. Gymfinity will follow procedures on personal cleanliness and communicable diseases in accordance with licensing rules and the guidelines for exclusion of children from child care as adapted from the Department of Health Services, Division of Public Health.

For any off-site injuries, first-aid supplies will be with each counselor. If it is an emergency requiring a call to 9-1-1, they will be called and dispatched to the location of injury and the parents will be called with the details. Gymfinity will never be at a location where they will not have immediate access to assistance should 9-1-1 need to be called.
Gymfinity will report all communicable diseases, when required, to the regional licensing office of the Department of Children and Families, to the local health department, and to parents of all enrolled children.  Parents of all enrolled children will be notified when their child has been exposed to an illness other than a communicable disease.
We have received training in first aid.  We follow standard emergency medical procedures for treating injuries.  A head injury will be treated as a serious injury, and parents will be notified as soon as possible. Camp Director’s have a current certification in child cardiopulmonary resuscitation (CPR) including training in the use of an automated external defibrillator (AED). Each head-counselor will carry their own first aid kit when the children are off site.

Superficial injuries will be washed with soap and water and covered with a bandage or treated with ice.  Parents will be told about the minor injury when the child is picked up at the center or delivered to the parent or other authorized person.

Gymfinity staff will fill out an accident report the day of the injury occurring. The parent will be contacted the day after the injury occurs to check on the status of the child, and will then fill out a follow up report. These reports will be in the child’s file.
 
If there is a need for emergency medical treatment, 9-1-1 will be called and the child will be taken to the hospital or urgent care center noted on the camp registration form if possible. If a specific care center cannot be chosen, they will be taken to University Hospital (UW Hospital and Clinics) located 600 Highland Ave, Madison, WI 53792. Should an ambulance be needed, parents will be responsible for any costs incurred. Parents will be contacted as soon as possible after contacting 9-1-1.  Gymfinity will request that your child be taken to the emergency medical facility that you designated on the child enrollment form, unless distance or time are a factor in attaining care.

Camp counselors and directors will wear gloves when dealing with bodily fluids or open wounds. Gloves will be used one time and properly disposed of in a garbage can out of the reach of the children.

All medication administered, accidents or injuries occurring during the time the child is in Gymfinity’s care will be noted in a binder kept onsite or in possession of the camp’s directors if travelling off-site. Any marked changes in behavior or appearance, and/or any observation of injuries to a child's body received outside of our care will be entered into the center's medical logbook.  As a licensed child care provider, we are required to report suspected child abuse or neglect to the local authorities.

Hand washing and toileting facilities shall be provided and accessible to children. Hand wipes will be provided by camp counselors and the director if a sink is not provided for handwashing after using the restroom or before and after meals.

Children will not be in direct contact with animals during camp unless taken to a facility that specializes in animal/human contact like a petting zoo where all the animals are healthy and safe. Kids are able to touch the animals but must immediately wash their hands or wipe their hands with hand wipes carried by staff at all times.
All children must be independently potty-trained. Children should have a change of clothes in their possession when attending camp as it is a camp requirement. If a bathroom-related accident does occur, the child must change into their other reserve clothing. If they do not have an extra change of clothing, the parents will be billed appropriately for clothing from our pro-shop to ensure the child is in clean clothes the rest of the day.

Mild Illness:
a. Gymfinity has not been authorized by the licensing agency to provide care for mildly ill children.

b. Medications
Gymfinity will administer medications under the following conditions.  Prescriptive and non-prescriptive medication will only be given to children if parents have completed the authorization form provided.  All medicine must be in its original container bearing the label with child's name, dosage and administration directions. Gymfinity will not exceed the age-related dosage on the label of any medication without a written doctor's authorization.  Blanket authorizations, such as dispensing pain relievers at Gymfinity’s discretion, is not allowed.

Bee-sting medication, inhalers, an insulin syringe, EPI-pen, or other medication or device used in the event of a life-threatening situation may be carried by a child over the age of 7 years with written authorization from the parent and the child’s physician.
 
Prior to applying sunscreen or insect repellent to a child, Gymfinity will obtain a written authorization from the child’s parent.  The authorization shall include the brand and the ingredient strength.  All sunscreen should be provided by the parent/guardian.

c. Smoking
Smoking is not permitted on the premises of the center during licensed child care hours.

VI. Nutrition: Policy Effective: May 1, 2022
Gymfinity will follow USDA guidelines when planning our menus.  Parents providing their own children's meals and snacks will be informed IN WRITING of the USDA nutritional requirements.

No child will go without nourishment for longer than 3 hours.  Gymfinity will offer the following meals and snacks to all children in attendance at the times identified in the daily schedule.
A.M. snack (approximately 9:30am)
Lunch (a nutritious lunch must be provided by parents every day except for Friday) (between 11:30am-1:30pm)
P.M. snack (approximately 2:30pm)

Parents must provide their children with a sack lunch Monday through Thursday. If a child forgets his or her lunch, Gymfinity will call the parent and discuss options. The parent is able to then bring a lunch for their child. If the parent decides that Gymfinity should find and provide lunch for their child, Gymfinity will do so but charge the family for the cost of the food provided.

If a child has special dietary needs (whether due to a medical condition or personal choice) or has food allergies, parents must notify Gymfinity in writing and provide the twice daily snacks for their child.

Any special diet based on a medical condition, excluding food allergies, but including nutrient concentrates and supplements, may be served only upon written authorization of a child’s physician and upon the written request of the parent.

Weekly records of snacks are available for your review upon request. All snacks are nut-free.
Gymfinity requests that parents send their child with a reusable water bottle to fill up and drink from frequently during the day. The kids will always have access to water/water fountains, etc. Gymfinity staff will ensure that kids are drinking enough water to stay hydrated throughout the day during activities and warm weather.

VII. Daily Activities: Policy Effective: May 1, 2022
Gymfinity does not include religious instruction or practices in any daily activities.  We do not offer prayers before meals and snacks.  We do not offer a religious education program or curriculum.

Gymfinity does not celebrate any religious holidays.
 
Gymfinity plans activities according to the age and developmental level of each child in care, and provides children with a variety of experiences.  The daily activities include a flexible balance of indoor and outdoor activities, active and quiet play, including individual and group activities.  The activities provided will expose the children to a variety of cultures, encourage the children to use and develop language and literacy skills, use large and small muscles, think creatively, learn new ideas and skills, and participate in imaginative play.  The activities are designed keeping in mind the risk of excess fatigue and over stimulation but with an emphasis on ensuring that each child can be successful and feel good about himself or herself.  All activities are safe; none include the use of firearms, archery, etc. Some of the activities include:

Language development:  i.e., Books, music, scavenger hunts
Large muscle skills:  i.e., Balls, hula hoops, swinging, outdoor play, swimming, activities in the gym
Small muscle skills:  i.e., Arts / crafts, science experiments, drawing
Creative expression:  i.e., arts / crafts
Self-help skills:  i.e., dressing self for swimming activity, scavenger hunt, team activities with working together
Literacy skills:  i.e., Books, scavenger hunts
PLAY is the major component of our program.  Enough time, materials and space will be provided for children to actively explore the world around them.  Children will have an opportunity to use a variety of art materials and manipulative and housekeeping equipment.

Children will go outdoors daily when weather permits, so children should be dressed appropriately for the weather.  

The children may be kept indoors during inclement weather such as any of the following:
Heavy rain
Periods of thunder or lightening
Temperatures above 90 degrees Fahrenheit
Wind chills of 0 degrees Fahrenheit or below
Wind chills of 20 degrees Fahrenheit or below

Outdoor play space:
There is not an outdoor play structure on the premises of the center.  Children will be able to play on Gymfinity’s grass area. Any activities outside at Gymfinity are planned and regimented to adhere to the week’s theme. Trampolines and inflatable bounce surfaces on the premises may not be accessible to or used by children as a part of the camp curricula.

Swimming pools:
The center will be using an off-premises pool, wading pool, water attraction or beach for the children.  Whenever we utilize any of these off-premises, water-oriented facilities, Gymfinity will follow all safety and supervision requirements as specified by licensing rules.

If a swimming facility does not have a certified lifeguard on duty, one of our camp counselors will be a lifeguard.
There will be at least one counselor per 25 kids at the pool.

Kids will be told of the rules prior to the trip; no rough play, climbing on the shoulders of another child, pushing a child under the water, etc.

Kids who wish to swim in the deeper area will be required to swim to a specific point and back safely and with efficiency if they wish to be able to swim in areas reserved for good swimmers. Kids are not required to do the test and can stay in a shallow area, but again, those who wish to be in the “deep end” or in an area reserved for efficient swimmers must test with a lifeguard on the property or with the counselor who is certified as a lifeguard. They will be subject to this, as our requirement, as well as any required skill demonstrations required by the activity site. Their results will be noted in their files.

Camp counselors are assigned to specific kids which they must keep an eye on and monitor, doing frequently headcounts and attendance checks. It is requested that a headcount be executed every 30 minutes.
There will be a roster to check kids in and out of the water, to know who is in the water versus on dry land. The waterfront supervisor will be on dry land will be monitoring the kids coming in and out, keeping track of every child’s whereabouts.

If a child participating in water activities cannot be found, all of the kids in the pool will be removed from the pool while the lifeguard or water front supervisor begin a search for that child within the water. While the lifeguard(s) is searching, the camp director will call 9-1-1.

At the conclusion of swimming time, children will change back into dry clothes prior to getting on the bus to return to camp. A female counselor will make sure the female changing room is empty, and a male will make sure the male changing room is empty. When they are empty, the children will collectively go inside and change while a counselor stands guard at each door to ensure strangers do not enter the changing room with the kids inside. Children should be old enough to change in and out of clothing without the help of a counselor as a counselor cannot go inside the changing room while the children are using it and changing.

Periods of rest:
Rest or nap time will be provided for all children younger than five years of age who are in care for more than four consecutive hours.  Children who do not sleep may get up after 30 minutes, and children who awaken early will be allowed to get up when they awake. Gymfinity will always provide awake children with appropriate activities.
Gymfinity will allow children to watch G-rated television including VHS or DVDs.  Children may not bring VHS or DVDs from home.  The children will be allowed to watch television under the following situations:  during rest periods, during lunchtime, or after lunch to rest before resuming activities.  Children are not required to watch television, and other activities will be available during that time for children to use.

Field trips: We take field trips multiple times a week to locations which will be communicated to parents at the time of registration.  Emergency information for each child will be taken whenever the children leave the premises. Field trips are subject to change without notice. Field trip schedules are considered tentative until the final schedule is released in May, 2021. Parents/guardians are responsible for monitoring the website for any changes in field trips and contacting Summer Camp Director with any questions regarding changes. Gymfinity and Bummer Free Summer Camp do not guarantee any field trip or itinerary.

VIII. Child Guidance: Policy Effective: May 1, 2022
Children's behavior will be guided by setting clear limits or rules for children.  Gymfinity staff will talk with children about expected behaviors and model those behaviors consistently for them.  Gymfinity staff will state positively what children can do, using specific terms (i.e., "you need to walk" rather than "don't run").  Undesirable behavior will be redirected to another activity.  Children will be given a wide variety of age appropriate activities to choose from and will be given the attention they need before they demand it.  Behavior management will be for the purpose of helping children develop self-control, self-esteem and respect for the rights of others.

We understand that there will be times when a child will become distraught, fussy or may have difficulty with emotions.   Our first action in these situations will be to attempt to determine the cause of the distress.  It may be related to a basic need such as hunger or comfort, or it may be that the child just needs some extra time and attention. We understand that crying may be normal.  At these times, staff will stay calm and will do whatever they can to soothe the child.  Sometimes this may mean just allowing the child to cry for a few minutes and then trying again.  However, there also may be times when staff may need parental advice or assistance, and we won’t hesitate to call parents if we feel that it is necessary.

Time outs:
Gymfinity uses “time outs” to deal with unacceptable behavior.  A "time out" or "take a break" may be used when other techniques have not been successful.  A time out will be used to remove a child from a situation that has gotten out of control before the situation can escalate.  Time outs can never exceed five minutes and will not be used with children under three years of age.  When used, the timeout will immediately follow the behavior.  Staff will stay with the child and talk about the unacceptable behavior, and provide solutions and suggestions to alternative behavior.  Rather than use a specific time-out chair or corner, we will have the child "take a break" near the others so the emphasis is on relaxing or cooling down rather than isolation and punishment.  The child will be praised after completing the timeout and will be assisted in rejoining the group.

We recognize that no single technique will work with a child every time.  If a child exhibits unacceptable behavior, our staff will request a conference with parents to consider how to deal with the behavior.  If the behavior continues, the next steps may include referrals to appropriate community resources and / or discharge of the child from care.
In accordance with DCF 252 Licensing Rules for Day Camps, actions that may be psychologically, emotionally or physically painful, discomforting, dangerous or potentially injurious are prohibited.  Prohibited actions include spanking, hitting, pinching, shaking, slapping, twisting, throwing or inflicting any other form of corporal punishment on the child; verbal abuse, threats or derogatory remarks about the child or the child's family; physical restraint, binding or tying the child to restrict the child's movement; enclosing the child in a confined space such as a closet, locked room, box or similar cubicle; withholding or forcing meals, snacks or naps; actions that are cruel, averse, humiliating or frightening to the child; or punishing a child for lapses in toilet training.  These forms of punishment will never be used, even at a parent's request.

IX: Transportation: Policy Effective: May 1, 2022
Gymfinity does provide transportation to and from field trips.  We contract for transportation services for activities held off-site.  Contracted transportation services are provided through Go Riteway located 501 Verburg St, Cambridge, WI 53523. They can be reached at 608-423-2225.
Parents will be notified in advance of the date, time and destination of any field trip requiring transportation.
Children will never be left unattended in any vehicle. A specified camp counselor will check the bus to ensure everyone has left the bus upon arriving to a field trip location or back to the camp premises.

In order to track children being transported and ensure that their whereabouts are documented from the time the child is picked up until that child is relinquished to the responsible caregiver, Gymfinity staff will take a written attendance checklist to make sure that all children are accounted for by name and sight at each transition, including each time any vehicle is exited.  If we are transferring the children to another responsible adult, staff will be sure the adult acknowledges the transfer before leaving the child.

All vehicles will be visually checked upon reaching a destination to ensure all children have exited.  Any vehicles that have a manufacturer seating capacity of 6 or more passengers in addition to the driver have a child safety alarm that is in working order.  This does not include public transportation.

All vehicles used to transport children in care are covered by liability insurance.
Annually, Gymfinity will obtain and review the driving record of any person who will transport children and obtain an inspection for each vehicle used to transport children.  This does not include public transportation.

Gymfinity is able to transport a child with a disability. The bus used for camp will have a wheel chair lift for on and off the bus if a parent lets Gymfinity know that it is needed. On a day  a child with a disability is present, two staff members will be assigned to assist that child with getting on or off the bus; either by way of the wheelchair lift or the stairs should that child be able to use stairs. Those two staff members are also responsible for getting that child off the bus in case of an emergency; by carrying/lifting, etc.

Gymfinity will receive permission from parents for each field trip the child is set to attend during the camp week with a specific form to be signed and due before the start of camp. Parents will receive an e-mailed itinerary of the week’s activities a few days before the start of that camp week to ensure parents know where kids will be at all times.

Our counselors will be prepared during all field trips with the medical journal, a first aid kit, proper Red Cross CPR tools, attendance rosters, and contact information for parents in case of an emergency. They will also have the medications of children who may need their medication during the trip.

XI: Emergency Procedures: Policy Effective: May 1, 2022
Fire drills shall be practiced with the children monthly.  In addition, tornado drills shall be practiced with the children monthly during the tornado season which is April through October.  Completion of all practice drills will be documented.

In the event of a lost child, Gymfinity staff will check all areas of the center.  If the child cannot be found, the child's parents and / or emergency contact and the police will be notified immediately.  We will notify the Department within 24 hours after the occurrence.

In the event of a tornado warning, if ample time is allowed, children will collect in the gym in the main floor area and if there is not time and an immediate response is demanded, coaches will get the attention of the children and instruct them to move against the gym’s interior back wall.   Blankets, a portable radio and flashlight, with extra batteries for both are on hand in case of a tornado.  The attendance form and emergency contact information will be brought along.

In case of an emergency that would require an evacuation, children will be evacuated through the nearest safe exit.  The attendance form and list of phone numbers for parents and emergency contacts will be taken along to ensure that all children are accounted for and all families notified.  Children will be assembled at Ten Pin Alley, the bowling lanes across the street from Gymfinity.

If we are unable to re-enter the building after a necessary evacuation, we will take the children to Ten Pin Alley (6285 Nesbitt Road) and contact Jason J. Orkowski, Gymfinity’s owner, to begin contacting all parents.

If the center should lose the use of heat, water or electricity before the center opens, we will notify parents of camp attendees to apprise them of the situation. If the loss of services will be temporary we will continue to hold activities provided there would be no danger to the children. If the outage is deemed as more long term, we would close the activities for the period and provide refunds or make-up sessions to each registered camper.  

If the center should lose the use of heat, water or electricity while children are in attendance, staff will determine if activities may be done safely. If the loss of services will be temporary we will continue to hold activities provided there would be no danger to the children,. If the outage is deemed as more long term, we would close the activities for the period and notify parents that an early pick up would be required. For a longer term halt in activities (over 1 day) we will issue refunds or make-up sessions to each registered camper. If the interruption is less than one full day, no refund or make up will be issued.

In the event that local schools close due to severe weather, the center will close.  Parents will be contacted to pick up their children within the hour.

In the event that the center receives a threat to the building or its occupants (i.e., bomb threat, bodily injury threat, etc.), law enforcement and parents will be immediately contacted to advise them of the threat.  Depending on the nature of the threat, evacuation and/or closure or lockdown may be required.

 All staff are aware of the protocol for the above emergencies and can execute them.
